For the secret levels:
Listed below are the whereabouts to all the entrances that lead to the super secret levels:

SECRET LEVEL			HOW TO GET THERE

Launch Facility		In episode 1, access the level toxic dump, and make way to the 
			level exit. In this last room, search about the walls for a 
			crack, Shoot the crack and a secret level entrance will appear.

Spin Cycle		Enter episode 2 mission 5 called occupied territory and make 
			your way to the level exit, dont just exit yet though. Turn 
			around on the exit bridge and look up to the control room. If 
			you look closely, you will see a switch, shoot it and a secret 
			door will open to the left, Jump to this new area and hit the 
			button to warp to spin cycle.

Lunatic Fringe		In episode 2, mission 8, Dark side, make way for the slime cave, 
			complete with obelisk and blast the marked wall to reveal a 
			secret area holding the switch that accesses the level lunatic 
			fringe.

Tier Drops		In episode 3, mission 5, movie set, touch the USA sign located 
			at the space station to set open a secret panel that holds the 
			button leading to the level Tier drops.

Freeway			In episode 3, enter the level known as hotel hell and jump 
			through the waterfall. Inside you will find a teleporter. 
			jump in the teleporter and blast the flaming pots to get to the 
			freeway.
